For Individual Membership
Thank you for your interest in joining the
Polish American Congress.
Please read these instructions in their entirety before you
proceed. |
1. After reading the instructions, bring up the Membership Form by
clicking "Go to the Form" below the instructions.
2. Fill out the form on the screen.
3. Print the form on your printer. It should be only one page. If it
overflows, try resetting the font to a smaller size, or adjusting the
margins of your page. Margins should be 0.75" or 0.5" on all
four sides.
4. Sign the form where indicated.
5. Look up the Polish American Congress State
Division or area where you live, and mail the form to the
President or Membership Officer (if listed) of that Division.
6. The Division will contact you and inform you about dues and other
requirements.
7. (If there is no Division in your state, you may choose to join as
an individual through the PAC National Office in Chicago. In that case,
send the application to Polish American Congress, Mrs. Pamela Komorowski,
5711 N. Milwaukee Ave., Chicago, IL 60646. She will inform you about
dues and other formalities.)
